The Lindy Nursing Scholarship

The Lindy
Nursing Scholarship will be to award the applicant who meets the following
requirements:
- Is a resident of Montgomery, Green
or Warren County, Ohio.
- Is enrolled in an accredited School of Nursing in Ohio
- Has a record of academic
achievement (GPA 2.9 of higher on a 4 point scale) and submits the
following materials:
- Written proof of enrollment in a
qualifying school (2, above)
- A copy of academic records to
verify achievement (3, above)
- A written and signed statement of
financial need
- A letter from the candidate’s
faculty advisor verifying enrollment, achievement and need
- A written and signed statement
from the candidate of his/her intent to practice in Montgomery, Greene or Warren County upon completion of studies, graduation
and certification.
- Complete home and campus
addresses of the applicant, including telephone and social security
numbers
All materials
should be sent in a single envelope by U.S. Mail to:

All entries mailed in time to arrive at
the address no later than July 7, 2005.
The
scholarship provides for a one-thousand dollar ($1,000.00) stipend made jointly
payable to the recipient and the school in which the person is enrolled.
The Lindy Nursing Scholarship
honors the memory of Mrs. Linda Huesman Lamb, a heart transplant recipient,
activist for heart disease causes, wife and mother who died in 1991.
All applicants
will be notified by mail. The award will be presented at the Huesman Heart Open
golf outing on Thursday, July 21, 2005. Te decision of the judges is final.
